Why Assessment Matters in Massage Therapy
Historically, many people are used to coming in for a massage and getting straight into hands-on treatment within minutes of walking through the door. But what if I told you there was a more effective and safer approach to treatment? This is where orthopaedic assessment comes into play.
In recent years, massage therapy programs across Canada have increasingly shifted towards more evidence-based assessment and treatment approaches, and I was fortunate to be trained within this newer curriculum. Pre and post-treatment assessments are essential for several reasons, including client safety, treatment effectiveness, and in guiding the therapist’s clinical decision-making process.
Safety
I frequently work with clients who are post–motor vehicle accident, post-injury, or post-operative, and understanding how to perform and apply comprehensive assessment techniques is critical for both safe treatment and appropriate referral when necessary.
Certain medical conditions can potentially be aggravated by massage therapy if proper screening is not performed. Assessment allows therapists to identify red flags, contraindications, and situations that require referral to another healthcare professional.
For example, symptoms such as swelling, throbbing pain, redness, warmth, or tenderness in the lower leg may indicate a condition such as a deep vein thrombosis (DVT) but may also be confused with a severe strain of the gastrocnemius or a tear of the Achilles tendon. While no single orthopaedic test can definitively diagnose a DVT, recognizing the signs and performing proper clinical screening can help identify between a strain of one of your lower leg muscles or a potentially life threatening medical condition that requires referral and further care.
Without proper assessment training, a therapist may proceed with aggressive manual therapy when treatment should instead be postponed pending medical evaluation. A strong understanding of assessment, symptom presentation, and contraindications helps ensure that client safety always remains the top priority.
Treatment Effectiveness
Pain is incredibly subjective, which is why objective assessment measures such as range of motion testing, strength testing, posture analysis, and palpation can be extremely valuable before treatment begins.
How many times have you told a healthcare practitioner that your left shoulder hurts, only for assessment to reveal that the primary issue may actually originate elsewhere? The location where pain is felt is not always the true source of dysfunction. Referred pain, compensatory movement patterns, joint restrictions, muscular imbalances, and nerve involvement can all contribute to symptoms presenting away from the underlying cause.
Assessment helps direct treatment toward the structures and movement patterns most likely contributing to your pain. Rather than providing only temporary symptom relief, this allows for a more individualized treatment plan aimed at achieving longer-lasting results.
Evidence-based assessment also allows therapists to measure progress over time. Reassessing range of motion, pain levels, strength, and functional movement both before and after treatment helps determine whether the chosen treatment approach is effective and whether modifications are needed moving forward.
In addition to hands-on treatment, assessment findings can help guide individualized home care recommendations, including stretching, strengthening, mobility work, and activity modification to support long-term recovery and injury prevention.
